Starting weight loss medication is a significant step — and knowing what to expect can make the journey smoother and less stressful. Here’s what most patients experience during their first month.

Week 1: Your first dose is typically the lowest available — this is intentional. Starting low allows your body to adjust gradually. Most patients notice a subtle decrease in appetite within the first few days. Some experience mild nausea, which usually resolves within a day or two. Stay hydrated and eat smaller, more frequent meals.

Week 2: By the second week, the appetite reduction becomes more noticeable. Many patients describe it as ‘food noise’ quieting down — they think about food less, feel satisfied with smaller portions, and find it easier to make healthy choices. This is the GLP-1 medication working on the hypothalamic appetite centers in your brain.

Week 3: Some patients see early weight loss results by week three, typically 2–4 pounds. More importantly, you’re likely establishing new eating patterns. Your provider may ask about your experience at a check-in to assess how you’re tolerating the medication.

Week 4: By the end of the first month, most side effects have stabilized or resolved. Average weight loss at one month is typically 3–5 pounds, though individual results vary significantly. Your provider will evaluate whether to maintain or adjust your dose at your follow-up.

What to eat: Focus on lean proteins, vegetables, and whole grains. Avoid greasy, high-fat foods that can worsen nausea. Eat slowly, stop when you feel full (you’ll notice you feel full much sooner), and prioritize protein to preserve muscle mass.

Common side effects in the first month include mild nausea (most common, affects ~40% of patients), decreased appetite, occasional constipation or diarrhea, and mild fatigue. These are typically manageable and improve with time. If any side effects are severe or persistent, contact our medical team immediately.
